Complete the form below to request a chemical waste pickup.
Allow 3-5 business days for requests to be fulfilled.
Instructions
1. Begin by entering your information in the Requester Info section.
2. Enter the location of the chemical waste in the Location section. If you have waste in multiple locations, you will need to submit requests for each location.
a. In Campus, Building, or Outdoor Area, enter your building then select your room number. If you have additional details enter them in Additional Location Details. If your room number does not appear in the drop-down list, enter it in Additional Location details.)
3. Enter the chemical information by clicking on Add a Container.
4. In the Add Container window include the following information:
a. Count: enter the number of containers. If you have multiple containers with the same type of contents enter the number here. This will create multiple entries for you. If you have multiple containers with different contents, enter it as a different container by clicking on Add a Container at the bottom of the form.
b. Container Type Description: enter the container the waste is stored in (ex. 16 oz plastic bottle, 1 liter glass bottle). If none enter None.
c. Container Contents: Include the composition of the waste and percentage. If you don’t know the percentage leave blank.
d. Under HazWaste Determination, select Hazard Level type. If unsure choose Undetermined.
e. Waste Characteristics: Select a listed characteristic if it applies to your waste, otherwise leave blank.
f. Waste Codes: Enter waste codes that apple to your waste. If unknown, leave blank.
g. Custom Fields, Hazard Class: select the waste hazards.
h. Documents: you can include pictures or additional information, such as Safety Data Sheets (SDS).
5. Click on add container.
6. For the following actions, click on Actions for the specific container and select the appropriate item:
a. If you want to duplicate your container click on Duplicate Container.
b. If you need to make changes to your container select Edit Container.
c. If you want to generate a label you can select the label size. A new window will appear with your container label. Note: When you submit your request you will be given the option to download a container label.
d. To delete a container click on Delete Container.
7. When you are done and ready to submit your request, click on Submit Pickup Request.
a. Click Save if you are not ready to submit but want to save your form.
8. You will receive an email confirming your pickup request was submitted.
